The deadline to submit the completed form is May 15, 2025 — and it’s quickly approaching.

⚠️ Important: To remain eligible for the July 2025 distribution of funds, the form must be submitted by May 15, 2025. Counties that do not submit the form by the deadline will forfeit their allocation.
